Modern healthcare has come a long way from the days when you visited a doctor only when you were truly ill. Today, healthcare is about your wellbeing at all stages of life, during times of sickness and health.
MHC patients receive a wraparound plan of care that supports them when they are well and during periods of illness. You won’t find yourself needing as many hospital visits or appointments with other services outside MHC because issues are identified and can be dealt with before they become a problem.
With Client-led Integrated Care (CLIC) you decide on how often you need to see us and what services you need. We believe this is a more empowering and nurturing healthcare model that puts you in charge.
We work with you to develop a personalised care plan that sets health goals, shows you ways to look after yourself and keeps to a minimum any health complications that may affect you.
Here’s how it works. Every few months you and the CLIC nurse allocated to you discuss your progress and go over your care plan. We also keep in touch with you to provide additional support when you need it outside of these consults.
Our Client-led Integrated Care is extremely affordable because you get four FREE appointments with your nurse each year. Should you need more, they are only $24. Doctor’s visits are at our standard rate of $42, or $19 if you have a community services card.
