Modern healthcare has come a long way from the days when you visited a doctor only when you were truly ill. Today, healthcare is about your wellbeing at all stages of life, during times of sickness and health.
The Client-led Integrated Care programme (CLIC) is a community-based health care initiative for people with long term conditions designed to help you gain control over your health by providing you with the support you need to better self-manage.
Eligible MHC patients receive a wraparound plan of care that supports them when they are well and during periods of illness. One of the aims of this programme is to help reduce hospital visits or appointments with other services outside MHC because issues are identified and can be dealt with before they become a problem.
You will meet with a Registered Nurse, who completes a FREE 1 hour assessment with you covering a range of health issues. After the assessment, the Nurse will refer you the Health Coach who will assist you to develop a personalised care plan that sets health goals, shows you ways to look after yourself and keeps to a minimum any health complications that may affect you. You will then follow up with the health coach or your CLIC Nurse as needed. We also have a Pharmacist available on site who can complete a review of the medications you are currently taking. The pharmacist will then feedback any suggestions with both you and your doctor.
The CLIC programme allows you decide on how often you need to see us and what services you need. We believe this is a more empowering and nurturing healthcare model that puts you in charge.
The initial assessment with the nurse is FREE, as are any follow-up appts required with your CLIC nurse. Appointments with the Health Coach and our onsite Clinical Pharmacist are also free.
Any other appts you might have with a nurse will be at the standard consultation cost. Visits with your Dr are not included in this assessment and will incur the standard consultation fee.
