Changing your doctor is easy but there are a few formalities to go through.
If you wish to become a new patient of MHC please call reception to find out which GPs are taking on new patients. Read about our doctors.
A slightly longer appointment will be needed for your first visit, during which registration paperwork will be completed and information will be collected and so the fee for an initial appointment is slightly higher.
You will be asked to sign a registration document to allow MHC to request your previous medical records so your new doctor can provide the best possible care for you.
If you ever decide to change to a GP at another health centre, we will be happy to forward your notes for you. All we need is your signature giving us permission to do so, together with the name of your new GP.
Please note that under the Privacy Act 1993 and the Health Information Privacy Code, when a family’s records are sent to another GP, each member of the family over the age of 14 needs to sign a release form.
