Mornington Health Centre Nurse's Clinic operates as both a
support team for our General Practitioners and as separate
clinic performing many specialist services. For
routine nursing services you need to make an appointment through
the main reception.
Patients requiring urgent attention do not need an appointment.

Nancy Todd is the Maori/Pacific Island Health Nurse who can assist you and your family/whanau to receive the best possible care. The Maori/Pacific Island Service is a mobile nursing service that can visit in the home, workplace or hospital setting and offers a range of services to help keep you in good health.
This service is available to patient who are enrolled with the Mornington Health Centre, if you are not and would like to find out more information please feel free to contact Nancy at the Health Centre or on her mobile 0274 367572.

All
children prior to starting school are invited to attend
a well-child check.
This includes measuring their height
and weight, and assessing their development, nutrition,
dental hygiene, and immunisation status. To have this
check, please make an appointment with both a nurse
and your doctor.
This check is no longer being
done by the public health nurse at schools.
All nurses run well child checks although Rachel, Pauline, Candice and Maree have a special interest in young children

All
diabetic patients will be offered an free annual diabetic
check. Patients will be notified by post when this is
due.
This check
involves assessment of diabetic control and ensures
regular retinal screening and diabetic foot screening.

Patients can also visit the clinic for education of how to manage their diabetes.
Maggie Hanton and Julie Clarke run these clinics. Maggie also runs a diabetes prevention programme, which is aimed at people who are at risk of suffering from type 2 diabetes. |

The aim of this clinic is to help patients manage their asthma.
Patients will be offered two visits, the first of which covers inhaler techniques, peak-flow monitoring, and lifestyle issues.
The second visit involves updating patient action plans.

Cathy Modrich runs this clinic. Any patients attending this clinic can make an appointment.
Also Cathy may carry out alternate 6 monthly reviews for asthma medication in consultation with your GP.

We can offer both individual and group support to people who would like to stop smoking. This programme is based on the Cancer Society's "Can Quit" programme and involves motivational interviewing. We are also able to supply nicotine replacement therapy. Liz Medlin and Ingrid Howard run these clinics
Cardiovascular Risk Assessment
We are screening all patients within defined age groups for cardiovascular risk. This involves a 30 minute nurse’s appointment where risk is assessed and management/ lifestyle advice is provided

Chris Lyall and Martine Shaw provide care to patients that have a mild mental illness such as depression.
They provide an early intervention service that involves assessment, treatment and follow up evaluation. Patients can be referred from their GP, practice nurse or self refer.

Appointments can be made through Reception.
The role is funded from a Ministry of Health Initiative and there is no patient charge to see Chris or Martine.
Chris works full time and Martine works 1 day a week |
